Impact of obesity on outcomes in myocardial infarction combating the "obesity paradox".

چکیده
Overweight and obesity are increasing in epidemic proportions both in the United States and throughout the world (1). Alarmingly, the proportion of patients with severe and morbid obesity is increasing even more so than are overweight and obesity per se. Almost all of the major coronary heart disease (CHD) risk factors, including lipid disorders (especially elevated triglycerides and low levels of highdensity lipoprotein cholesterol), glucose abnormalities, the metabolic syndrome, and diabetes mellitus, hypertension or left ventricular hypertrophy, and physical inactivity, are all adversely affected by overweight and obesity. Additionally, overweight and obesity may be independent risk factors for CHD and have adverse impacts on other cardiovascular (CV) disorders that may accompany CHD, including heart failure (HF), atrial fibrillation, and risk for sudden cardiac death (1).

Impact of Age on Risk Factors and Clinical Manifestations of Acute Coronary Syndrome: Observations From the Coronary Care Unit of Sulaimani, Iraq
Background: ST-segment elevation myocardial infarction (STEMI) and non-ST-segment elevation myocardial infarction (NSTEMI ) are common types of acute coronary syndrome which are associated with the risk factors of age, obesity, hypertension, and diabetes. Objective: The present study aimed to examine the effects of age on the risk factors and clinical sym...
متن کاملLack of “obesity paradox” in patients presenting with ST-segment elevation myocardial infarction including cardiogenic shock: a multicenter German network registry analysis
BACKGROUND Studies have associated obesity with better outcomes in comparison to non-obese patients after elective and emergency coronary revascularization. However, these findings might have been influenced by patient selection. Therefore we thought to look into the obesity paradox in a consecutive network STEMI population. METHODS The database of two German myocardial infarction network reg...
متن کاملEvaluation of the impact of statin therapy on the obesity paradox in patients with acute myocardial infarction
The phenomenon of obesity paradox after acute myocardial infarction (AMI) has been reported under strong recommendation of statin therapy. However, the impact of statin therapy on this paradox has not been investigated. This study investigated the impact of statin therapy on 1-year mortality according to obesity after AMI. A total of 2745 AMI patients were included from the Korea Acute Myocardi...
متن کاملThe Influence of Body Mass Index on Outcomes in Patients Undergoing Cardiac Surgery: Does the Obesity Paradox Really Exist?
PURPOSE Obesity influences risk stratification in cardiac surgery in everyday practice. However, some studies have reported better outcomes in patients with a high body mass index (BMI): this is known as the obesity paradox. The aim of this study was to quantify the effect of diverse degrees of high BMI on clinical outcomes after cardiac surgery, and to assess the existence of an obesity parado...
متن کاملMeasures of obesity and outcomes after myocardial infarction.
Overweight and obesity have become increasingly common; worldwide, at least 1.1 billion adults are overweight and 312 million are obese, when overweight and obesity are defined conventionally as having a body mass index (BMI) of 25 kg/m and 30 kg/m, respectively.1,2 In the general population, overweight and obesity are associated with increased risk of developing cardiovascular disease,3,4 and ...
